Corrections
Every listing should match FDA's record. Here is where each kind of fix is made.
For a listed company
- Your name, address, establishment type or devices. These come from your registration and listing with FDA, so change them there (see FDA's device registration and listing page). We check openFDA every week, and the change reaches this site with FDA's next export.
- A page that misreads your FDA record, such as a device on the wrong facility, two facilities merged that should not be, or a facility missing: send a correction with the form below. Include the page address and your FDA registration number, and we will check it against FDA's data.
- Certificates such as ISO 13485. We show none yet. When we do, a certificate will show as "company-stated" when it comes from your own website, and as "verified" only after we have found it on the certification body's register. You can tell us about one with the form below.
For anyone else
If a number, name or date on MedtechSourcebook doesn't match its FDA source, or a page says something it shouldn't, use the form below. Each registration number links to FDA's own record, so the source is one click away.
